Exposure and Response Prevention (Ex/RP)

Exposure and Response Prevention (Ex/RP) is an evidence based cognitive behavioral treatment for Obsessive Compulsive Disorder (OCD). OCD features a pattern of unwanted intrusive images, impulses, thoughts and fears (obsessions) that lead to repetitive behaviors or rituals that serve to “undo” or neutralize the anxiety related to the obsessions. The goal of Ex/RP is to reduce OCD symptoms (including anxiety that fuels obsessions and compulsions) and help the client learn long term management strategies for OCD.

Common themes that cause distress for people who struggle with OCD include contamination, fearing harming someone, needing things to feel “just right”, doubting and having difficulty with uncertainty, scrupulosity (irrational fear that one is immoral or going against religious beliefs), relationships, order and symmetry, unwanted sexual thoughts, fears of losing control (may be accompanied by unwanted aggressive/violent thoughts), perfectionism, magical thinking (linking unrelated actions or events such as “good” or “bad” /”lucky” or “unlucky” colors, numbers, etc.) and health issues. Compulsions may include washing and cleaning, checking, counting, arranging and ordering, following a strict routine, repeating words/phrases/prayers/behaviors, seeking or demanding reassurance, “confessing” or disclosing.

Treatment of OCD with Ex/RP generally consists of 15-20 individual therapy sessions that are about 90 minutes each. The therapist helps the client with gradual and systematic exposures to obsessions or objects, situations, mental images/thoughts, or other stimuli that trigger anxiety and obsessions. The therapist also works with the client to reduce or eliminate responding to obsessions with compulsive behaviors or rituals.

Many clients find that once they have learned Ex/RP and have experienced positive results, they are able to use Ex/RP strategies to manage OCD symptoms long-term.

Exposure based treatments are also available to treat anxiety, phobias, panic disorders and trauma.
